• List of your current doctors. By first & last names, type of

specialty like, oncologist, gynecologist, dermatologist and where they are located.

• List your current dentist. First name, last name and address.
• List of Prescription Drugs. Names of all medications and the dosages required for everyone in your household.
• Bring relevant cards if you are applying for a Medicare and/or Medicaid Advantage, Supplemental or Medigap Plan and Part D prescription drug coverage.
